The game deals with some very dark themes.
Stealth is improved in this game, and it also gives you the option of pacifism or violence. I'm usually a pacifist, but this guy is a Nazi so.... yeah, Imma slice his neck.

Like in the first game, light sources play an important role in Stealth and it is wise to remove lightbulbs and switch off lamps.

Like before, Artyom has a speech at the beginning of each level. You can also now pick up diary pages, on which Artyom will write further details. They aren't necessary, but help the plot greatly.

IR scopes are useful for targeting enemies in the dark without using a flashlight.

The mutants are uglier than ever. This is a watchman, those creepy howly things that run around on the surface.
Artyom is still haunted by flashbacks from the past. In this case, he witnesses the apocalypse first hand from the perspective of a passenger planes pilot.
Lightning is a new hazard in some levels. Watch for burning trees.

Friendly stations are a welcome break from action, stealth, and surface sequences.
