NecroVisionN: Lost Company takes place before the first game and puts the player in the shoes of its main antagonist Jonas Zimmermann at the time he was just a guy sent to the front lines to find a vaccine to a mysterious disease decimating the troops at the Western Front during World War I.
As its predecessor, NecroVisioN: Lost Company is a straightforward shooter. The gameplay hasn't changed much and still requires the player to mow down hordes and hordes of zombies with his vast array of WWI-weaponry from the first game as well as six new weapons, as well as some special vampiric weapons like the ShadowHand which makes its return. The ShadowHand requires adrenaline to work which is earned by performing combos on enemies. The player also needs to have unlocked the deadly spells he wants by beating the corresponding challenge before the ShadowHand does anything useful. If enough adrenaline is available, the ShadowHand also allows to resurrect fallen enemies and lets them fight on your sight for a short while. Also back and playing an integral part of the game is the gas mask which obstructs the view but saves the players lives once the deadly poisons start spreading.
To spice up the fighting, performing combos will grant the player benefits like a short slow down of time after two headshots. After a combo has been successfully executed, the Fury timer starts. Doing another combo during this time fills up the Fury meter and allows for unleashing even more powerful demonic powers. To increase the amount of Fury the player can collect and use, he needs to find hidden artifacts during the course of the game or by going once again into challenge room.
The challenge room not only gives the player direct access to each individual campaign level that he has already finished but also contains several challenges which unlock after finishing a level in the normal campaign. These challenges include, for example, shooting 50 zombies while they are in the air and keeping flying corpses from hitting a bridge. Successfully completing these challenges then grants the player access to new spells for the ShadowHand or will add a weapon permanently to the players inventory.
In addition to the ten missions long single-player campaign, during which the player also takes over the control of a FT17 tank and a Halberstadt CL.II airplane to fight the really big monsters (including a few bosses), the game features multiplayer for up to 8 players. The majority of the maps are from the first game with only three new ones waiting. The game modes still include Deathmatch, Team Deathmatch, Capture the Artifact (a CTF-variant) and Last Man Standing. New is the game mode "Gas Attack".

Trivia
German version
There are a number of changes to the German version:
- All blood effects were removed or replaced with grey smoke. Gore effects like cut off limbs were completely removed.
- All killed enemies disappear instantly. Because of this the necromancy skill is rendered useless.
- The efficacy of the kick attack was greatly reduced
- Various death animations were removed
- The flame thrower weapon was removed.
- The combo displays on screen were removed.
