Elizabeth Find M.D.: Diagnosis Mystery is a hidden object game with a medical theme. Most of the time, the goal is to find and click on all the items from a list at the bottom of the screen, on locations cluttered with random paraphernalia. The list sometimes shows the object silhouette or sounds instead of words. Some items are written in red, indicating the need to use one of the medical tools on someplace in the scenery to acquire them. The medical bag tab contains a thermometer, a petri dish, cotton swabs, a reflex hammer, a syringe, a penlight and the stethoscope.

Standard variations of gameplay make an appearance: in some stages, the screen is split in two similar images, and the player has to click on the differences. In some places, the objective is to collect several instances of the same object. In some screens, the goal is to put back pieces of the scenery.

Occasionally, mini-games appear where the player must perform some medical activity, like a maze game with lines of contrast dye going to organs for radiology examination, inoculating cells on a petri dish and performing blood type tests.

The red cross hint button highlights the position of one of the required objects, creating a glowing halo around it. It takes time to recharge after use. Wrong clicks remove several seconds from the timer. There's an option at the beginning of playing without the time limit.
